Who completes it
FirstService Residential, Nevada, LLC is the contact of record for Vistana Condominium Owners Association, Inc. in the register of active associations (NRED-ACTIVE-HOA), published by the Nevada Real Estate Division and read on August 15, 2026. A lender questionnaire for a unit in this association is addressed to the association through that office.
Where the contact of record is a management company, that office usually completes the form itself. Where it is a law firm, an accountant or a registered agent, the form is normally passed to whoever keeps the association's books. In neither case is it answered by the lender, the buyer or the seller.
Which form the lender sends
For a condominium the lender sends Fannie Mae Form 1076, which Freddie Mac issues as Form 476, the Condominium Project Questionnaire. It travels with the addendum, Form 1076A or 476A, covering building safety and deferred maintenance. Many lenders send their own version instead, asking the same underwriting questions in a different order.
Two things that were true until recently and are not now. The short form questionnaires, Fannie Mae Form 1077 and Freddie Mac Form 477, are retired. So is Limited Review: for applications dated on or after 3 August 2026, Full Review is the regime, which means the full questionnaire is answered even on the loans that used to escape it.
The answers that hold a file up are usually the same few: the split between owner occupied and rented units, how much of the budget goes to reserves, whether a single owner holds more than the permitted share of the units, litigation, and any deferred maintenance or special assessment.
What Nevada sets by statute
A lender questionnaire is not the statutory resale package, and the two are routinely confused. The questionnaire is the lender's own form and is generally neither capped nor timed by statute. The resale package that goes to the buyer is both, and in Nevada it runs as follows.
- Who prepares it
- the association may charge the unit's owner a reasonable fee to cover the cost of preparing the certificate (NRS 116.4109)
- Current charge
- $213.84 for the resale certificate and $190.73 for a demand statement under the Real Estate Division's current schedule, the statutory bases of $185 and $165 growing by CPI up to 3 percent a year; an insurance-proof item joins the resale package July 1, 2026
- Update or rush
- $100 expedite, sooner than 3 business days, on either document
- Deadline
- 10 calendar days for both; miss it and the purchaser does not owe the delinquent assessment
- Who pays
- The unit's owner, expressly, at the owner's expense
